In general, I think your screening question is fine. It makes me wonder what I would use, but I get impressions, so it's different.

One thing strikes me about what you wrote: "I am making all decisions about myself, my family, my life, my career, and my home". Doesn't that seem like ... alone, so why have a screening question in the first place?
